History : Stray. Arrived last night as possible UO/FLUTD, blood running down tail. Grit and mucus plugs coming from urethra, followed by free flowing hematuria. P received SQ LRS, simbadol, and monitoring urine output in Medical. Overnight staff said p has been urinating, passed large amounts of mucus plugs, urine has been bloody.

MM tacky with delayed skin turgor, ~7% dehydrated
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: nsf
PLN: No enlargements noted
H/L: NSR, NMA, CRT 2 seconds, Lungs clear, eupnic
ABD: Large semi-firm bladder, uncomfortable on light palpation. Tried to gently express bladder, small amount of hematuric urine expressed.
U/G: MI. Penile tip is inflamed and irritated, no grit or mucus plugs seen.
MSI: Ambulatory x 4 but prefers lateral recumbency, skin free of parasites, no masses noted. Dirty urine soaked hair coat
CNS: depressed mentation
Rectal: normal externally

Urinary unblockage : pass u-cath, empty and lavage bladder, leave indwelling u-cath (confirm placement with with AXR) and hook up to urinary collection kit, monitor ins/outs and empty and quantify urine output q4hrs
Simbadol 0.6mL SQ SID x 7 days
Prazosin 1mg PO BID x 7 day
Recheck blood work tomorrow 3/4/18
Recommend urinary diet for life
