DVM Intake Exam Estimated age: Reported 5 years old – exam is consistent with this Microchip noted on Intake? Scanned negative History : No hx given Subjective: Mewed and hissed in carrier. Netted for exam – growled throughout exam. Objective BAR-H, BCS 6/9 EENT: Large superficial ulceration on nasal planum. Pt licking nose frequently. Eyes clear. Oral Exam: Not performed due to behavior and pt being in net. H/L: NSR, NMA Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: URI Plan: Move to isolation, start doxycycline 1.2 ml PO SID x10 days Prognosis: Good SURGERY: Temporary waiver due to URI. 1088
