Medical Summary 05/10/17 DVM Intake Exam Estimated age: 3 yr Microchip noted on Intake? No. History : Surrendered by owner. No medical concerns noted. Subjective: Observed Behavior – Shy at first, then warms up with petting, especially likes to be rubbed around his ears and face. Relaxes and allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist. HR=180, RR=32. BCS=5/9. ORAL: Mild gingivitis. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Male, neutered. MSI: Ambulatory x 4, no lameness. Flea dirt and slightly thin fur over caudal dorsum. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Mild gingivitis, flea allergy dermatitis. Otherwise apparently healthy. P: Applied Advantage. Rec. monthly flea control. Excellent prognosis.
