06/10/17 19:19 Tiger is a 15 year old spayed brown tiger shorthair cat. He was adopted from the ACC 10 years ago. He has no know injuries or health problems. Tiger is very outgoing and friendly with strangers. He has not lived with any children, cats, or dogs so his behavior with them is unknown. Tiger has not bitten or scratched another person.

KNOWN HISTORY: Previously lived with: Adults Behavior toward strangers: Outgoing and friendly MEDICAL BEHAVIOR: Screams when trimming nails, resistant to restraint but tolerates rest of exam ENRICHMENT NOTES: 06/12/17 Lying in back of kennel with ears forward and tail wrapped around body. He rolls over onto his side when spoken to, eyes soft. Allows petting along head and body, standing up and leaning in with shoulders and head. Begins to purr. So sweet, cute and round. Doing well! EVALUATION: Cage Condition: Cage is neat Reaction to assessor: Tiger was relaxed and lying down peacefully at the back of the kennel. Reaction when softly spoken to: Tiger meows loudly then comes forward and solicits attention. Reaction to cage door opening: Tiger moves over to the other side of the double kennel. Reaction to touch: Tiger rolls around and rubs up against all the objects in the kennel. He leans in when pet, paces back and forth soliciting attention and has a ton of energy. He occasionally whips around and lip licks when touched along her back but doesn’t display any further reaction. His body is relaxed, he enjoys being pet all over and he rolls around playfully, reaching out for attention. Reaction to being picked up: Tiger was a bit tense when picked up but allows all handling. ACTIVITY LEVEL: Lively VOCAL: Somewhat chatty CHARACTER TYPE: Affectionate, Sweet, Playful BEHAVIOR SUMMARY: Average Tiger interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. No known history of behavioral problems. This cat is showing behavior appropriate for new or experienced cat parents.

Mod dental dz Plan -retro (negative), vaccines, pyrantel, activyl -Recommend blood work and -Vetcheck tomorrow to monitor urinations overnight and make sure not developing UO since so reactive to bladder palpation -Simbadol x3 days (painful bladder and neck mass) -Clavamox x 7 days due ot purulent debris on neck mass Prognosis: Fair to good SURGERY: Neutered
