Medical Summary DVM Intake Exam Estimated age: 6weeks Microchip noted on Intake? none History : Subjective: Observed Behavior – allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P =160 R = 24 BCS 4.5 EENT: OU- shut closed with thick, dried purulent d/c, no nasal discharge noted; once cleaned off dried d/c, globes both appeared intact with moderate conjunctivitis, but OS appears microophthalmic Oral Exam: pink gums PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, unable to palpate testicles MSI: Ambulatory x 4, flea dirt, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities offered canned kitten food- ate two large bites, then stopped Assessment flea dirt conjunctivitis OS- may be compromised- may not be visual Plan ofloxacin drops OU BID x10 force feed, check app Prognosis: good; may not be visual in OS long term, but still can lead great quality of life SURGERY: Temporary waiver due to underage and conjunctivitis
