Medical Summary DVM Intake Exam Estimated age: 2-4 years based on overall appearance Microchip noted on Intake? Scanned negative History : Stray, no hx given Subjective: Alert, reluctant to come out of carrier prior to exam or go into carrier afterwards. Allows most handling but has tense body. Objective BCS 5/9, MMs pink and moist, CRT < 2 sec EENT: OS: moderate chemosis and the palpebral conjunctiva are purple and bruised (bulbar conjunctiva are pink and injected). Moderate blepharospasm, moderate mucoid discharge. Normal fundic exam. Skin above eye is excoriated and alopecic. OD: Eye itself is normal, with no conjuncrtivitis, no discharge, no chemosis etc. Skin above eye – between right eye and right ear – is alopecic and erythematous with multiple lacerations. AU: small amount black granular discharge. Nose: clean, no discharge. Oral Exam: Clean teeth H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Ambulatory x 4, skin free of parasites or scabbing from flea bites, no masses noted, slightly dry hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment 1. Wound above right eye – R/O self trauma due to ear irritation vs. other cause 2. Left eye – R/O externally-inflicted trauma and secondary conjunctivitis Plan: 1. Cleaned ears and administered ivermectin 0.1 ml SQ during exam, repeat ivermectin in 10-14 days 2. Erythromycin OS BID x10 days, monitor conjunctiva 3. Onsior 0.4 ml SQ SID x3 days Prognosis: Good SURGERY: Temporary waiver due to conjunctivitis. 1088
