Medical Summary DVM Intake Exam Estimated age: 6 weeks Microchip noted on Intake? no History : found stray, no medical history Subjective: QAR, euhydrated, one kitten produced #6 diarrhea prior to exam – parvo SNAP negative Observed Behavior – a little nervous but allow all handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: all deciduous teeth, no ulcers PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated, distended and gassy intestines, loose feces on fur U/G: female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: diarrhea – start ponazuril 0.3 mL PO SID x 5 days (first dose given today) Plan: ok to find foster prior to surgery, then adopt Prognosis: good SURGERY: Temporary waiver due to underage, diarrhea
