DVM Intake Exam Estimated age: 8 yo Microchip noted on Intake? no Microchip Number (If Applicable): History: na Subjective: BARH Observed Behavior – Timid, allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 120 R = 24 BCS 7/9 EENT: Right eye has corneal scar with vascularization and possible synechia, left eye wnl, ears clean, no nasal or ocular discharge noted Oral Exam: moderate dental tartar/gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Neutered male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: Old injury to right eye (minor significance at this time). Tafi was nervous during intake but allowed for minimal handling by counselor. He allowed her to collar him and pet him gently on the head, his eyes were wide and pupils dilated.

Basic Information:: Tafi is an 8 year old, DSH male cat who was neutered prior to coming into the shelter. He was adopted from BACC in the beginning of January and was returned about 3 weeks later due to his behavior. He has no known health issues or injuries and last saw a vet when he first entered the shelter.

How is this cat around strangers?: Tafi is shy and hides from guests when they enter the home.
How is this cat around children?: Tafi hides from children and is not interested in playing with them.
How is this cat around other cats?: Tafi has not interacted with other cats.
How is this cat around dogs?: Tafi has not spent time around dogs.
Behavior Notes: Tafi’s owner has not bathed or groomed him in the home. He does not enjoy being picked up or held and struggles to be placed in a carrier.
Bite history:: Tafi has no bite history but has scratched a friend when he tried to take Tafi out of a cabinet

For a New Family to Know: Tafi is described as being shy, fearful and mellow. He keeps to himself most of the day and has been an indoor cat. He eats both wet and dry food, is litter box trained using an open box with clumping or crystal litter, and likes to scratch on cardboard or carpet. He will play with toy balls and strings, and sleeps under the bed or couch.

MEDICAL BEHAVIOR:: 01/29/18 Nervous but allowed all handling without resistance or trying to scratch/bite
ENRICHMENT NOTES:: 01/30/18 Lying in litterbox with a low, tense body, face partially hidden behind box and tail curled around feet. Makes eye contact when spoken to. He lifts his head and sniffs the air when I place treats down next to him, ears shifting forward. Comes halfway out of box and begins to eat treats. He allows petting along his head and body while rolling over onto his side and pulling his feet and tail closer to his body. Begins to purr and tilt ears. Conflicted behavior – needs more time to adjust. Has potential to warm up. Ate treats from hand. 01/31/18 Lying in back of kennel, body tense. Tolerates petting along head and body while turning head to watch my hand and beginning to scent mark the litterbox’s edge. Raises rear as my hand passes along his back and rolls over onto his side. Very sweet, appreciates attention once approached. Remains in back of kennel where he feels most comfortable – grabs treats with his mouth and pulls them towards the back of his kennel. Purring loudly towards end of session. Showing improvement daily. 02/01/18 Lying in back of kennel with a low body, tail wrapped around feet. Rolls over onto side when spoken to and begins to scent mark kennel floor. He leans in for petting when touched and continues to scoot and roll around the back of his kennel. Timid and sweet. Begins to eat food after a few more moments of petting. Purrs loudly. He turns his head and nips my hand with moderate pressure halfway through session. Seems to be more of a sign of excitement opposed to aggravation – body remains soft as he nips and he continues to solicit attention afterwards. Has potential to become overstimulated. Warming up daily. 02/02/18 Cage was very messy. Lying down in the back of the kennel, tail wrapped around his body. Curious, leans forward to investigate. Rubs his cheeks against the kennel furnishings. Accepts petting on his cheeks, gentle nudges my hand. Very food motivated- inched towards the front as soon as he smelt the tuna. He seems a little unsure of his surroundings, but is very sweet. Needs more time. 02/03/18 At the back of the kennel, lying down behind the litter box. He tilts his head and rubs his cheeks against the litter box. Scent marks and inches forward when coaxed. His body perks up and he gives gentle head butts when pet. Purrs very loudly. His tail flicks and he quickly whips around and vocalizes when pet for too long along his back. Possible overstimulation. Doing well! Really coming out of his shell.
Cage Condition:: Cage is neat
Reaction to assessor:: Tafi is lying in his litterbox with a low body as the assessor approaches, head lowered and ears peeking out.
Reaction when softly spoken to:: Tafi begins to purr loudly when spoken to.
Reaction to cage door opening:: Tafi lifts his head and chirps as the cage door opens, then licks his lips.
Reaction to touch:: Tafi allows petting along his head and body while leaning in and head-butting the assessor’s hand. He continues to purr and rolls over onto his side. He then hisses and turns to nip the assessor’s hand with moderate pressure continued petting. Continues to solicit attention after cage door is closed.
