History : Subjective: Observed Behavior – hissing, allowed exam using towel wrap, nervous Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = R = BCS 4/9 EENT: ears clean AU, conjunctivitis OS, no nasal discharge Oral Exam: deciduous dentition, no oral ulcers, mm’s pink/moist PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, no alopecia CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment ~4-5 week old FI kitten diarrhea conjunctivitis Plan parvo tested on intake – negative start terramycin OU BID x 10 days start panacur (100 mg/ml): 0.3 ml PO SID x 5 days start ponazuril (50 mg/ml): 0.3 ml PO SID x 3 days start nutrical 2 ml PO BID x 5 days recheck 8/21 – CTM for URI signs, diarrhea progress Prognosis: Good SURGERY: Temporary waiver due to underage
