Oral Exam: severe dental dz and significant halitosis and “chattering” even on sedated exam, 5-8y
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: fi – no scar
MSI: sedation precludes ambulation skin free of parasites, no masses noted, healthy hair coat, decreased muscle mass
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: na
Assessment: 5-8y fi dmh

Prognosis: good to excellent with dental treatment/extractions
Plan:0.33 cc convenia sq and 0.44 cc simbodol due to oral pain
