DVM Intake Exam intact male Estimated age: 5 weeks Microchip noted on Intake? negative History : Patient is stable. No vomititng, diarrhea, coughing or sneezing Subjective: Apparently healthy kitten Observed Behavior – Meaowing, moving, very active. Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: toothless PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Apparently healthy Plan feed several times a day Eating by himself. Prognosis: good Temporary waiver due to underage/underweight.
