Medical Summary DVM Intake Exam Estimated age: ~5 weeks old Microchip noted on Intake? scan negative History : Subjective: Observed Behavior – friendly Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = — P = — R = — BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: deciduous dentition, no oral ulcers PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment ~5 week old intact male kitten apparently healthy eating well on his own Plan OK for foster until big enough for neuter vs. New Hope placement Prognosis: Excellent SURGERY: Temporary waiver due to underage/underweight
