DVM Intake Exam
Estimated age: 2-4 weeks
Microchip noted on Intake? no
History: Stray kitten found in someone’s yard.
Subjective: Kitten is QAR
Observed Behavior – Allows all handling. No “warm up” time needed – immediately seeks comfort of handler.
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
T = NA P = wnl R = wnl BCS 3/9
EENT: Eyes crusted shut, severe chemosis OU, globe is white OS; ears clean; no nasal discharge noted
Oral Exam: healthy deciduous dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: wnl
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: not performed
Assessment: Neonate <4wks orphaned, Conjunctivitis (severe)
Plan: Recommend rescue placement ASAP as kitten is ill and not eating on her own. Syringe fed gruel – continue q2-4 hrs. Starting on Doxycycline 0.05mls PO q24h x 10d – gave 1st dose. Flushed OU with eye wash and applied Erythromycin – continue eye wash prn and erythromycin ophth oint – thin strip OU 2-3 times daily for 10 days.
Prognosis: Fair
SURGERY: Temporary waiver due to underage/underweight and illness.
