Medical Summary DVM Intake Exam Estimated age: 3-4 years Microchip noted on Intake? no History : stray, unknown Subjective: BAR, H Observed Behavior – Tolerates all behavior, does not resist PE, brushing, blood draw, vaccines, etc. Very sweet cat Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NP P = 200 R = eup BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild calculus caudal cheek teeth otherwise clear PLN: No enlargements noted H/L: NSR, Grade I/VI L parasternal HM, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: 2 desc testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NP Assessment 3-4 year old M DSH 1. Very soft, focal HM Plan -FVRCP, rabies -Retro -MC -pyrantel -flea treatment -OK to set up for surgery and make available for adoption Prognosis: Excellent SURGERY: Okay for surgery
