GONE 6/21/18 SPERRY is a 15 year old kitty who was found outside and brought to the shelter. She is emaciated and dehydrated and has a cold. Sperry needs some follow up care and a good home.

DVM Intake Exam

Estimated age: ~15yrs based on intake exam.
Microchip noted on Intake? scanned negative by LVT on intake. MC placed on intake.

History : Stray cat found outside and brought in.

Subjective / Observed Behavior – QAR, allows all handling. Slightly depressed mentation

Evidence of Cruelty seen – none
Evidence of Trauma seen – none

Objective
BCS 2/9
EENT: Eyes have mild mucoid discharge, sunken in; ears clean, no nasal discharge noted; mild audible congestion
Oral Exam: no teeth. gingiva appears healthy
PLN: No enlargements noted
H/L: No murmur ausculted; CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: female, intact.
MSI: Ambulatory x 4 but paretic, skin free of parasites, no masses noted; haircoat is unkempt, slightly dirty. focal 1cm crust along dorsal head. prolonged skin tent
CNS: Mentation is slightly depressed
Rectal: grossly normal.

URI — suspect viral with 2nd bacterial

Prognosis:
fair to poor

Plan:
Move to med iso
IVC placement
LRS fluids 10ml/hr
CBC / chemistry
Doxycycline 50mg/ml — 0.54ml PO q24h x 10 days
nebulization q24h x 5 days
fortiflora
Vitamin b12 injection SQ 0.25ml PO one time.
terramycin ophthalmic ointment — 0.25inches OU q12h x 5 days

SURGERY:
permanent waiver due to geriatric status
