Subjective: BARH
Observed Behavior – Hissed and tried to strike in the cage. Was netted and sedated for PE
Evidence of Cruelty seen – no
Evidence of Trauma seen – yes
Objective
P = wnl
R = eupneic
BCS 6/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: adult dentition with mild dental staining, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: MI, 2 testicles descended
MSI: Ambulatory x 4, skin free of parasites, no masses noted, there is a ~2cmx3cm full thickness wound on dorsal head with a healthy granulation bed and scabs forming, full thickness wound on dorsal neck ~5cmx2cm with healthy granulation bed and scabbing
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Wound on head and dorsal neck r/o self inflicted from allergies vs contact wound vs other
Plan: Continue to monitor while at BACC
Clipped and scrubbed wounds on head and dorsal neck
Clavamox 13.75mg/kg PO BID x7d
Recheck wound in 3 days
Simbadol 0.24mg/kg SQ once
Telazol 0.1ml IM for PE and wound cleaning
Prognosis: Good
SURGERY: ok to schedule for surgery

Preliminary Assessment:animal has two wounds, one on the dorsal scapula area and another on top of head.
Plan: DVM exam 10/20/17 and rabies
