SAFE 7/21/18 SMOKEY is a very handsome boy who was brought to the shelter. Person surrendering the cat stated her friend left this cat with her 3 days ago, he has been rubbing his penis and she thinks hair has gotten into it. Today she noticed that he was lethargic and bleeding. The vet at the shelter found he had a blockage and unblocked him and said Smokey needs a diet of wet food only. He will need further monitoring and follow up care.

History : Finder stated her friend left this cat with her 3 days ago, he has been rubbing his penis and she thinks hair has gotten into it. Today she noticed that he was lethargic and bleeding.
Subjective: Alert, mews a lot, walks out of carrier
Observed Behavior – Allows all handling including removal of plug from penis with my fingers, no complaining, struggling or tense behavior
Evidence of Cruelty seen – None
Evidence of Trauma seen – None
Objective
QAR, MMs pink, BCS 9/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: Clean adult teeth
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Soft, non painful, no masses palpated, not distended
U/G: Male intact, testicles S/S and scrotum matted and slightly crusty. Penis is partially exposed and there is a large amount of fur and white chalky discharge in the prepuce around the penis. There is a plug of white material visible inside the urethral meatus – it was removed and pt began to urinate immediately. Bladder is LARGE and full – expressed bladder with some difficulty, urine was orange.
MSI: Ambulatory x 4, skin free of parasites, no masses noted, recently shorn region just dorsal to the base of the tail. Fur smells strongly of cigarette smoke and cat urine.
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
Assessment:
1. Obese
2. FIC, urethral obstruction with large mucus plug in the tip of the urethra – unblocked during exam
Prognosis: Fair-good with appropriate care
Plan:
1. Recommend low-stress environment and high water consumption, feed canned diet only
2. Simbadol 0.9 ml SQ SID x7 days
3. Prazosin 1 mg PO BID x7 days
4. LRS 300 ml SQ SID x5 days
5. Gabapentin 0.7 ml PO BID x14 days
6. Recommend weight loss of 4-7 lbs
7. Urinalysis and urine culture – sending to IDEXX
8. Monitor daily to ensure pt is urinating. If pt becomes blocked again, this is an EMERGENCY and pt will need immediate treatment!!
