I came in the shelter as a STRAY on 11/10/2016 from NY 11237, owner surrender reason stated was STRAY. I came in with Group/Litter #K16-081062.

11/14/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 1.0 LBS.
11/14/16 Recheck conjunctivitis. O: QARH. Eating well, shy compared to sibling. No diarrhea. mm=pink, moist, CRT<2 s. EENT: OU – ointment present. Mild chemosis. OS – corneal opacification with central area of pitting and neovascularization. Globe intact. No nasal discharge. ABD: Mild pot-bellied appearance. Palpation WNL. MS: Amb x 4, mildly underweight. A: Resolving conjunctivitis. Healing corneal injury OS. P: Continue Terramycin x 5 more days. Excellent prognosis.

11/10/16 18:24 Skills is a white and black DSH kitten that is 6-7 weeks old. The kitten was found roaming the street with another sibiling. Skills allowed to be picked up and placed in a carrier. The kitten during intake was a bit fearful but allowed all handling. The kitten appeared to have URI.
