Observed Behavior – Closes eyes when head is touched, seems slightly tense.

BAR-H, MMs pink and moist, BCS 5/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: Hypersalivating. Mild tartar, moderate gingivitis.
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Soft, non painful, no masses palpated
U/G: Male neutered
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
Assessment:
1. Dental disease
2. Hypersalivating – R/O stress vs. herpes recrudescence vs. other cause
Prognosis: Good
Plan:
1. Vit B12 0.25 ml SQ once
2. Fortiflora SID x14 days
3. Recommend dental cleaning and daily tooth brushing
