DVM Intake Exam Estimated age: ~6yrs Microchip noted on Intake? no History : Owner surrender, history of bladder stones, cystotomy surgery in 2013 for stone removal, has been doing well on wet food only Subjective: BARH Observed Behavior – Hissing, growling, sedated with telazol 0.15ml IM Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = WNL R = WNL BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate dental tartar/staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Overweight, history of bladder stones Plan: Continue to monitor while at BACC. Simba was fearful during intake but allowed staff to collar and scan for a microchip. Simba became agitated and began to growl and hiss during intake. Staff was unable to take a picture of Simba on backdrop.

Basic Information:: Simba is a domestic short hair male cat who was surrendered due to his previous owner not being able to afford his care.
How is this cat around strangers?: Around strangers Simba is shy for a few minutes and plays exuberantly/somewhat rough.
How is this cat around children?: Simba has never been around children so it is unknown how he will react towards them.
How is this cat around other cats?: Simba has never been around cats so it is unknown how he will react towards them.
How is this cat around dogs?: Simba has spent time with larg and small dogs. Around small dogs Simba will often try to over power them through swatting and biting. Simba is relaxed and playful with large dogs.
Behavior Notes: Simba will often scratch on the walls, bed frame and rubber door mats. Simba is relaxed when riding in a car. Simba will struggle and hiss when being given a bath. Simba will attempt to swat and bite when having his nails trimmed.Simba enjoys being brushed, picked up/held and being placed in a carrier. Simba is not bothered when being disturbed while resting/sleeping.
Bite history:: Simba has never bitten a person but has bitten a small dog previously.

Medical Notes: Simba cannot eat dry food as he is prone to bladder stones.
For a New Family to Know: Simba is described as mellow, shy independent and playful. When at home Simba will follow around his owner and likes to play with stuffed toys and catnip toys. Simba was kept indoor only and was fed wet food. Simba is litter bos trained and uses an uncovered litter box with non-clumping litter.
KNOWN HISTORY:: KNOWN HISTORY: Lived Indoors Previously lived with: Behavior toward strangers: Shy for a few minutes and plays exuberantly/somewhat rough. Behavior toward children: Unknown Behavior toward cats: Unknown Behavior toward dogs: Spent time around a small and large breed dog. He was relaxed and playful with large dogs and would swat/bite the smaller ones. Bite or Scratch history: None Litter box training: Yes Energy level/descriptors: Mellow, shy independent and playful with a low activity level Other notes: He doesn’t like having his nails trimmed but enjoys being brushed, picked up/held and being placed in a carrier.
MEDICAL BEHAVIOR:: Hissing, growling
ENRICHMENT NOTES:: 12/04/17 Lying in litterbox with a low, tense body. Tolerates petting along head and body while turning head around to watch my hand, ears tilting and eyes squinted. Needs more time to adjust. Sprayed feliway. 12/05/17 Vocalizing softly in front of kennel. He steps forward to smell my hand, ears forward, and allows petting along his head and body while turning to smell my hand. Focused on environment – keeps pausing to peek outside into the room behind me. Open to interaction, but too aware of surroundings to completely focus on me today. Still in cat exam. Needs some more time to adjust. Sprayed feliway.
Cage Condition:: Cage is slightly re-arranged
Reaction to assessor:: Simba meows softly and was observing his surroundings at the front of the kennel.
Reaction when softly spoken to:: Simba sniffs through the bars, then rubs up against the cage door.
Reaction to cage door opening:: Simba lip licks and becomes alert.
Reaction to touch:: Simba leans forward to sniff the assessor’s hand, then closes his eyes and accepts gentle petting on his head. He perks up when pet along his body, gives head butts and starts purring. His tail sways slightly when pet but he doesn’t display any further reaction.
Reaction to being picked up:: Simba was a bit tense when picked up, but he remains calm and allows all handling. His tail thrashes around after a few seconds of being held, then he leaps back into the kennel.
