I came in the shelter as a OWNER SUR on 10/30/2016 from NY 10312, owner surrender reason stated was TOO MANY P. I came in with Group/Litter #K16-079015.

Silver came to the shelter surrendered by an owner with a limited profile. Upon intake, Silver was neutral and allowed handling. Reaction to assessor: Silver is hiding under the paper in her cage. Reaction when softly spoken to: Silver backs up, still hiding. Reaction to cage door opening: Silver hisses. Reaction to touch: Silver shifts away to avoid touch, but allows some petting. Placement Determination: Experience No Child Silver does not approach the assessor, appears tense and nervous, and allows limited handling. Silver is exhibiting behavior suited to experienced cat parents with no children.

S: recheck exam for URI; ringworm exposure; appears to have eaten some wet food O: QAR, hissing when approached decreased skin turgor mm’s pink; no nasal discharge seen OU- conjucntival hyperemia and mild clear mucoid discharge no murmur, regular rhythm abdomen- snp no ringworm lesions seen BCS 4-5/9 A: conjucntivitis, hx of nasal discharge- R/O URI vs other; currently being treated with erythromycin OU and doxycycline hx of ringworm exposure- fungal culture pending; currently getting lime sulfur dips dehydrated P: continue erythromycin, doxycycline, lime sulfur dips unitl culture results give 70 mls SQ LRS x 2 days
