Medical Summary DVM Intake Exam Estimated age: 1-2yrs Microchip noted on Intake? no History : Cat was left in ASPCA box outside of gate of BACC Subjective: Cat is BARH Observed Behavior – Immediately social, curious. Cat was active, but not fighting restraint for exam. Growled for vaccines and began escalating vocally and fighting restraint for blooddraw so aborted and will add to do at spay. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 170 R = 30 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Healthy adult dentition; Grade 0-1/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Apparently healthy Plan: Acceptable candidate for adoption or rescue pending behavior evaluation. Okay to schedule spay. CT when sedated for spay. Prognosis: Excellent SURGERY: Okay for surgery
