Your newly adopted animal is in treatment for an upper respiratory illness. The veterinarian is temporarily waiving this animal from the spay/neuter requirements of the City of NY until such time as the illness has resolved and the pet has sufficiently recovered. At that time, this animal must come into compliance with the spay/neuter requirements.

History: Finders stated that Chalky was a “building cat” that keeps trying to enter their apartment. Counselor asked what a building cat meant and they stated that the cat has been inside the building for two weeks now. They stated no one was caring for Chalky.
Chalky was nervous, lip licking, tail tucked, and keeping his body low but allowed handling.
Subjective: Cat is QARH
Observed Behavior – facing back of cage, licks lips, allows all handling with minimal restraint, keeps small does not try to flee
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
T = NA
P = 170
R = sniff
BCS 4-5/9
EENT: Eyes mild-moderate blepharospasm, epiphora and chemosis OU; ears clean, serous nasal discharge
Oral Exam: Grade 2/4 dental dz
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: intact male; both testicles descended
MSI: Ambulatory x 4, skin free of parasites, <5mm adenomatous mass above base of tail, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: not peformed
Assessment: Conjunctivitis/URI, Moderate dental dz
Prognosis:
Good
Plan: Gave 100mg Gabapentin PO on intake. Move to ISO. Start Doxycycline 50mg/mL – 1mL PO q24h x 10d; recheck day 7 and 10. Also start Erythromycin – thin strip OU q12h x 10d; first dose applied on intake.
