07/02/16 15:19 Seraphina is a black and white spayed five year old domestic shorthair. She was surrendered by her owner due to owner moving into a building that does not accept pets. She was a gift from a friend and was with her owner since she was a kitten. Seraphina has food allergies and is allergic to chicken, beef and seafood. Seraphina is described as fearful around strangers and takes several days to warm up to new people. She will engage in gentle play once she is comfortable. She has lived with two adults in the home and has not been around children or dogs. She lived with another cat and is described as respectful and will engage in gentle play. Seraphina was not bathed and does not mind when her nails are trimmed. She likes to be brushed and held. She does not mind when she is palced in a carrier. Seraphina is described as aloof, quiet and independent. Her activity level is described as medium and she lieks to stay in a favorite spot. She likes to sleep on the couch and has been kept only indoors. She used both a hooded and uncovered litetrbox and both crystals and clumping litter. She had a rough rope scratching post owner stated she used regularly. Upon intake Seraphina was tense and growled when carrier door was opened. During scanning she swatted at scanner, she was not collared and photo was taken inside carrier.

07/06/2016 BEHAVIOR EVALUATION – EXPERIENCE
Exam Type BEHAVIOR
Seraphina’s previous owners reports he is fearful around strangers and takes several days to warm up to new people. She will engage in gentle play once she is comfortable. She hasn’t been with children or dogs, but was respectful and will engage in gentle play with the other resident cat. She likes to be brushed and held, and does not mind when placed inside a carrier. She was described as aloof, quiet, and independent with a medium activity level. Reaction to assessor: Seraphina was inside a hidey den upon approach. Reaction when softly spoken to: Seraphina does not respond. Reaction to cage door opening: Seraphina remains motionless. Reaction to touch: Seraphina becomes alert and lowers her body, but sticks her nose forward to sniff the assessor’s hand. She was a bit hesitant and lowers her body, but allows all petting. She lip licks and wraps her tail closely to her body, but would stick her head outside of the opening and investigate her surroundings. She will peek out when coaxed, but appears to feel more comfortable inside the den. Placement Determination: Experienced Seraphina tolerates attention and petting but may be fearful or stressed in the shelter. She may be a little more independent, and may need time to warm up to her new home. We recommend that this cat go to a home with experienced cat parents.

Scan Postive # 985121007476651 S ~ 5 years old BARH Behavior- Gave 0.2mls Telazol IM EEN- clear and clean Teeth- Mild staining Coat- clean no flease or parasites seen Hair loss on outside of Left hock and also on the inside of both right and left hock. Possibly due to food allergies that the owner reported upon surrendering. Right hind foot 4th digits has a broken toe nail that occured during sedation.
