Medical Summary 4-26-17 Stray 6wk Male No MIC on Intake Subjective: BAR-H Observed Behavior – Friendly, allowed handling Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective 5/9 BCS EENT: Eyes clear, OD squinty no dc light sensitve, ears clean, no nasal discharge noted Oral Exam: NAD CVS: No murmur or arrhythimia ausc, Lungs sound clear ABD: Soft, no masses palpated U/G: M MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Not performed Assessment – Patient appears to be in good health Plan – Intake Monitor eyes for dc/uri, FVRCP postponed Prognosis: Excellent SURGERY: Temp wave – too young
