SAFE 7/20/18 SANDY was brought in by someone when owner died. She came in with housemates. SANDY is underweight and has dental issues. She also had some bloodwork results that show she needs further medical eval with a vet after adoption. SANDY needs a home asap where she can get all the care she needs.

PLAN: Strongly recommend dental cleaning and rads with extractions as needed. Pt will likely be much more comfortable after this and should have a much better appetite, and should gain weight!

Oral Exam: Severe tartar and gingivitis, halitosis; large chunks of tartar were cracked off the maxillary PM4s.
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Slightly tense, no masses palpated, not distended
U/G: Female, spay tattoo and ventral midline scar on abdomen
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate. Mild horizontal nystagmus OU at rest.
