Samuel A1115366 – 12 week old male orange tabby, stray:
6/15:
06/15/17
S/O: QAR/lethargic. ~5% dehydration. Food untouched but ate well with minimal coaxing. BCS 3/9
EENT: Pink mm, conjunctivitis, crusted bloody mucoid discharge OU, no oral ulcers
HL: Sneezing
INTEG: Small areas of alopecia on paws, flaky dermatitis on ears and face — Woods lamp positive
MS: Weakly ambulatory x 4
A: Underweight, URI, conjunctivitis, ringworm
P: Adding nebulization and force feeding q24 x 3 days, lime sulfur dip q72hrs, terbinifine 1.1ml PO q24 x 21 days. Good prognosis with appropriate treatment and management plan and appropriate response to treatment

Observed Behavior – difficulty seeing, handle with caution
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
T = P = 140 R = 20 BCS 4/9
EENT: eyelid adhesions right side (ankyloblepharon), bilateral conjunctivitis with 3rd eyelid exposure, nasal discharge with ulceration of nares
Oral Exam: pink mm w/ CRT <2s, deciduous teeth
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, stertrous breathing due to nasal congestion
ABD: Non painful, no masses palpated
U/G: male intact- diarrhea around hindend
MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: watery stool
Assessment
Severe URI
Conjunctivitis with eyelid adhesions right eye
Plan:
Antibiotics: Doxycycline 50 mg/ml- Give 0.25 ml by mouth once daily for 10 days
Terramycin opth ointment: OU twice daily for 7 days
Prognosis: Guarded
SURGERY:
