Medical Summary Sex: Female Scan: Negative Estimated age: At least 2 years as reported by owner Fleas: No flea dirt. Hx: Had kittens recently S: Meowing in cage, allows handling but tries to escape and will not stay still, indifferent to affection O: BAR-H, BCS 5/9, MMs pink and moist EENT: No discharge OU, AU, nose. Clean teeth. PLNs: Not enlarged. H/L: Grade 4/6 systolic heart murmur, equally heard on L and R. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Female, mammary glands slightly engorged A: 1. Heart murmur 2. Recent lactation Short-term prognosis: Good-fair Current surgery status: Permanent waiver due to heart murmur. P: 1. Administered Activyl for cats 2-9 lbs and pyrantel 0.7 ml PO during exam 2. Recommend echocardiogram to evaluate very strong heart murmur 1088
