Medical Summary 03/21/16 13:37 initial exam microchip scan negative approx 1 year old female cat icteric pinnae and mm’s dehydrated (~8-10%) heart and lungs WNL on auscultation conjunctivits and mucopurulent ocular discharge OU, 3rd eyelids up underweight BCS 2/9 abdomen soft and non-painful no masses palped amb x 4, no lameness or neuro deficits noted good strength, resists handling but is friendly prognosis: poor rec NH placement for work up and hospitalization (recommend full bloodwork +/- abdominal ultrasound for diagnostics and hospitalization on IVF/antibiotics/supportive care as needed) plan: attempted to get blood for CBC/chem/combo test – unable to get blood, try again tonight after on IVF placed IVC and start LRS + 20 mEq/L KCL @ 8 ml/r offer food and monitor appetite start erythromycin OU BID start baytril 0.4 ml SID x 10 days start famotidine 0.1 ml BID
