DVM Intake Exam Findings
History: stray.
S) Observed Behavior – Hissing and growling when trying to get cat out and then in net. Trying to bite.
Evidence of Cruelty seen – none.
Evidence of Trauma seen – none.
Scan – neg.
Age/ Sex? 6 m female.
O) BAR-H. BCS= 5/9. Very limited exam.
EENT: Eyes clear, ears clean, no nasal discharge noted.
Oral Exam: NSF when hissing/ growling at me.
PLN: No enlargements noted.
H/L: NSR, NMA, MMpk. Lungs clear, eupnic.
ABD: Too tense, trying to scratch/ bite.
U/G: Female. Unable to check for spay scar.
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat.
CNS: mentation appropriate – no signs of neurologic abnormalities
A) Healthy 6 mo F DSH
P) Ok to spay.
Microchip, vx, deworm, flea preventative, combo test.
Prognosis: good.
Weight 5.0
