Medical Summary DVM Intake Exam Estimated age: 2y Microchip noted on Intake? n History : stray Subjective: rodent ulcer type lesion on left upper lip and eosinophilic plaques (presumed) on right caidal thigh Observed Behavior – calm, allow exam Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 160 R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: clean teeth PLN: No enlargements noted H/L: NSR, NMA, , Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, moist skin lesion on caudal thigh, some purulent d/c CNS: mentation appropriate – no signs of neurologic abnormalities Assessment; EOGN lesion Plan gave convenia 0.5ml sq, dexamethasone 0.5mg once, will giev pred 5mg sid x 3 days then re-evalute for neuter Prognosis: good, nut may be a relapsing condition. recc hypoallegenic food SURGERY: Okay for surgery in house only
