Medical Summary DVM Intake Exam Estimated age: Microchip noted on Intake? no and couldn’t place because of behavior History : 1/2 cats surrendered by owner -Watson other cat who could no longer care for pets. Others re-homed. Subjective: Observed Behavior – Could handle without too much manipulation. Got out of carrier easily enough. When went to look at mouth-growling started, didn’t strike but extended front paws w/claws out. When tried to auscultate, distal tail smacking table. Evidence of Cruelty seen – none observed Evidence of Trauma seen – none observed Objective T = NE P = wnl R = wnl BCS 4/9 EENT: Eyes clear, ears clean-could not be evaluated (siblig ear mites suspected, no nasal discharge noted Oral Exam: mild tartar on cursory exam PLN: No enlargements noted H/L: none obvious growling and would not allow full exam NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: NE U/G: NE MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. BCS 4/9 lean CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: R/O healthy vs ear mites mild dental dz Plan: Went sedated for neuter: check ears>clean and tx prn abd palpation, oral exam Prognosis: good SURGERY: Okay for surgery
