Medical Summary DVM Intake Exam Estimated age: 6 months Microchip noted on Intake? no History : Stray, unknown Subjective: BAR-H Observed Behavior – Tries to flea, no aggression. Per handler, recommended to be netted so no handling was attempted outside of net. While in net, patient nervous but no aggression and seemed to allow all handling. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 180 R = eup BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: no performed PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: 2 descended, difficult to palpate bladder through MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NP Assessment 6 m old M DSH 1. Apparently healthy but very nervous Plan -MC -Retro – negative -FVRCP and rabies -flea treatment -pyrantel -consider behavioral assessment Prognosis: Excellent SURGERY: Okay for surgery
