9/18/16 Check geriatric cat O: BARH. mm=lt pink, moist. CRT<2s. Seeks attention, no touble handling. EENT: No oculonasal discharge. ORAL: Missing teeth. Mod to severe calculus, gingivitis. H/L: No murmurs. ABD: Soft, non-painful. Both kidneys feel mildly enlarged, smooth, non-painful. MS: Ambulatory x 4. Mod. muscle wasting over spine, thighs. A: Geriatric cat, est age at least 14 years. Dental disease. Bilateral renomegaly – DDX: neoplasia, pyelonephritis, other. Muscle wasting – DDX: age related, secondary to metabolic disease, neoplasia, hyperthyroidism. P: Rec placement with workup to assess health status more fully. Prognosis fair to good.

09/16/16 11:00 BASIC INFO: Princess is a brown tabby that was surrender due to home issues. Princess was initially found by the previous owners as a stray. Princess has never suffered any health issues or injuries with the previous owner. SOCILAZATION Princess is a very clam and affectionate cat. Princess is shy around stranger for a couple of minutes. Princess would eventually come out of hiding and greet whoever there. princess has lived in a home with another cat. Princess was super affectionate and playful with the other cat in the home. Princess and the other cat also slept in the same bed. Jerry has never interacted with any children or dogs. BEHAVIOR Princess has no pervious bite history with anyone or animals. Princess enjoys being held and brushed. Princess is frightened by loud noises, she will run and hide. Princess has never been given baths. Princess is mainly indoor , however, he likes to watch other cats outside the window. FOR A NEW FAMILY TO KNOW Princess is a relaxed and quiet cat. Princess was fed wet and dry foods 2x a day. Princess enjoys all toys and treats. Princess usually spelt in his cat bed. Jerry is fully litter box trained. Princess uses fresh step clumping liter in a uncovered litter box. UPON INTAKE: Princess was quiet but alert upon intake. princess allowed all handling.

Exam Type BEHAVIOR
Princess lived previously with adults and with another cat. She was affectionate and playful with the other cat. With strangers, Princess is initially shy but warms up. She has not been exposed to children or dogs. She is described as calm and affectionate. She was easy to handle at intake; she was a bit tense during her initial medical examination but allowed all handling and restraint without issue. During her behavior evaluation Princess was highly motivated to interact with the assessor, soliciting attention by vocalizing and reaching through the bars. Reaction to assessor: Princess is at the front of her cage, resting, when the assessor approaches. Reaction to cage door opening: Princess stands and approaches, vocalizing and rubbing on the bars as the door opens. Reaction to touch: Princess vocalizes and head butts the assessor’s hand. She is highly interactive, and she continues soliciting attention after the assessor steps away. Placement determination: Beginner Princess interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. She has no known history of behavioral problems. This cat can go to a beginner home.
