Medical Behavior Evaluation BLUE
Medical Summary Hx: Sneezing, dripping nose, congested, no app in 2 days, not much of anything in the litterbox. Owner has been syringe-feeding water. Apparently there was a clerical problem and the exam did not get saved into Chameleon, but the pt also came in on 6/13 with mild URI signs, and was given SQ LRS, mirtazapine, and Rx 0.7ml doxycycline SID x10 days. S: Active, energetic, allows full exam O: BAR, MMs pink and tacky, decreased skin turgor, estimated 8-10% dehydrated EENT: Creamy mucoid discharge from nose. Clean ears, eyes. Severe tartar and gingivitis; canine tooth recently fell out and client brought it in a plastic bag. H/L: NSR, NMA. Some sneezing prior to exam. Referred upper respiratory sounds. No increased resp effort. Abd: Soft, distended, no pain on palpation M/S/I: Amb x4. No skin lesions noted. UG: Female A: 1. URI, inappetent, dehydrated 2. Severe dental disease Short-term prognosis: Fair to good P: 1. Keep in Medical for 1-3 days 2. LRS 200 ml SQ SID x5 days 3. Doxycycline 0.7 ml PO SID x10 days 4. Nebulize BID x5 days 5. Monitor appetite 6. Long-term treatment needs to include dental cleaning, rads and extractions as needed 1088
