Medical Summary DVM Intake Exam Estimated age: Reported 10 years old – exam is consistent with this Microchip noted on Intake? Scanned neg – MC placed on intake History : Owner died. Came in with 2 other cats. Subjective: Calm, leans into head rubs and back scratches. Allows all handling. Objective BAR-H, MMs pink and moist, BCS 6/9 EENT: Eyes clear, mild brown waxy discharge AU, pt shaking head, small laceration in AD ear margin – scab accidentally removed and wound started bleeding. No nasal discharge noted. Linear collar-shaped region of hypotrichosis/alopecia around the neck. Oral Exam: Moderate tartar on remaining teeth. Many PMs missing. PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Slightly tense, no masses palpated U/G: Male neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Dental disease, many missing teeth 2. Ear discharge, head shaking – R/O ear mites vs. other 3. Geriatric Plan: 1. Cleaned ears and instilled Tresaderm during exam – start Tresaderm AU BID x10 days, clean q3 days x3 additional cleanings 2. Recommend dental cleaning. Prognosis: Good 1088
