DVM Intake Exam Estimated age: reported to be 2; age consistent with exam Microchip noted on Intake? neg; placed on intake History : owner surrender due to too many animals Subjective: BAR Observed Behavior – allows examination but struggles with restraint for CT-unable to perform today; no signs of aggression Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 5/9 EENT: Eyes clear, no nasal discharge AU-dark discharge; pruritic; suspect ear mites Oral Exam: mm pk, moist; CRT <2 sec; no significant tartar or staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact; 2 scrotal testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat with mild flea debris, no live fleas seen CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Ear mites Plan 0.1 ml ivermectin SQ now and again in 2 weeks treated with advantage Prognosis: excellent SURGERY: Okay for surgery
