Observed Behavior – Slightly tense body, allows all handling

Objective
BAR-H, MMs pale pink and moist, BCS 5/9
EENT: Moderate temporal muscle wasting. Moderate purulent nasal discharge. Eyes clear, ears clean, no ocular discharge noted.
Oral Exam: Severe tartar and gingivitis.
PLN: No enlargements noted
H/L: NSR, NMA, Slightly tachypneic, quiet lung sounds.
ABD: Soft, non painful, no masses palpated, not distended
U/G: Male neutered
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
Assessment:
1. URI – moderate, suspect viral with secondary bacterial infection
2. Severe dental disease, temporal muscle wasting
Prognosis: Good
Plan:
1. Move to isolation
2. Doxycycline 0.7 ml PO SID x7 days
3. Fortiflora SID x7 days
4. Vitamin B12 injection
5. Recommend dental cleaning and rads with extractions, then daily tooth brushing
