Litter box training: yes Other notes: Also, described Simba as affectionate with low energy that loves to follow you around and be petted. MEDICAL BEHAVIOR: 7/28/17- Subjective: Alert, comes to front and seems to appreciate petting ENRICHMENT NOTES: 7/26/17- Periodic meowing. Came to den window on approach, soft eyes and body. Stuck head through window when opened. Leaned into pets, purred, closed eyes. Ate offered treats. EVALUATION: Cage Condition: No change Reaction to assessor: Peter remains neutral, lying down on his cage bedding during the approach. Reaction to cage door opening: Peter stands with tail up, alert with soft eyes, ears erect and forward. Reaction to touch: Peter head bunts the assessor’s hand, purrs, and appreciates the petting on the head and body. Reaction to being picked up: Allows, but becomes antsy then walks back into the cage. BEHAVIOR SUMMARY: Average Peter interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

DVM Intake Exam Estimated age: 6-10 years based on overall appearance Microchip noted on Intake? Scanned negative History : Found outside, no health hx given. Soft formed stool with mucus in the litterbox. Subjective: Alert, comes to front and seems to appreciate petting Objective BAR, estimated 8-10% dehydrated based on decreased skin turgor, BCS 3/9, MMs pale pink EENT: Copious purulent nasal/ocular discharge covering entire muzzle. Eyes clear, ears clean. Oral Exam: Moderate tartar, gingivitis, halitosis. Seems to have some oral pain – pt jumps away from exam and palpation near left(?) mandibular ramus(?) H/L: NSR, NMA Lungs clear, eupnic ABD: Slightly distended, non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Ambulatory x 4, emaciated, generalized muscle wasting. Skin free of parasites, no masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment 1. Emaciated, dehydrated, muscle wasting – R/O diarrhea/GI disease vs. FeLV vs. leukemia vs. difficulty eating due to dental pain vs. other 2. URI 3. Oral/dental infection 4. FeLV Plan 1. LRS 150 ml SQ SID x5 days 2. Doxycycline 0.5 ml PO SID x10 days 3. Simbadol 0.3 ml SQ SID x5 days 4. CBC/chemistry 5. Nebulize BID x5 days 6. Consider other abx if necessary to treat oral infx (good anaerobic coverage, possibly clindamycin) Prognosis: Fair-poor SURGERY: Permanent waiver due to age 1088
