EENT: Ears clean, green mucoid nasal and ocular discharge noted
Oral Exam: Decidiuous teeth present
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Intact female, vulva UR
MSI: Paresis, skin free of parasites, no masses noted, healthy hair coat
CNS: Lethargic, dull – no signs of CNS defects
Rectal:

Assessment: URI

Prognosis: Fair

Plan:

1) Gave 25 cc SQ fluid
2) Start on 5ml/hr LRS for 24 hrs
3) Give 0.3cc Doxy-c SID for 14 days
4) Give 0.1 cc Cerenia SQ SID for 3 days
5) Apply Terramycine ointment in both eye BID for 7 days
5) Put in Med-Iso
