DVM Intake Exam

History : stray
Subjective: BAR, ~~5% dehydrated. Sneezing and congested and no interest in food.
Observed Behavior – very sweet. Easily handleable. Did well for all medical handling and procedures
Evidence of Cruelty seen – no
Evidence of Trauma seen – no

R = eupneic
BCS 3/9
EENT: Eyes have severe purulent d/c with severe chemosis, wiped globes clean but unable to fully assess globes due to chemosis-may be ruptured, ears clean, purulent nasal discharge
Oral Exam: kitten dentition, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic with audibly upper resp congestion
ABD: Non painful, no masses palpated
U/G: MI
MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment:
Underage
URI
Severe conjunctivitis-possibly ruptured globes
Dehydration
Not eating
Fleas
Plan: Continue to monitor while at BACC
Move to medical for close monitoring
LRS 20ml/kg SQ once-may need more tomorrow
Start doxycycline 10mg/kg PO SID x10d until 12/9-first dose given
Start terramycin OU BID x10d untul 12/9-first dose given
Cleaned eyes
Prognosis: Fair
SURGERY: temporary waiver due to illness and underage
