Medical Summary DVM Intake Exam Estimated age: 6 w Microchip noted on Intake? History : Stray – covered w fleas and “black pepper” – likely flea dirt Subjective: BAR Observed Behavior – Friendly Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = P = 200 R = eup BCS= 5/9 EENT: Eyes clear. Ears slightly dirty. No nasal discharge noted Oral Exam: normal deciduous teeth. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male. MSI: Ambulatory x 4, flea dirt seen but no live fleas, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done. Assessment: 6 wk male Fleas. Ears dirty slightly but NSF. Plan: FVRCP and other usual intake or 6 wk. Topical Frontline applied. Cleaned ears. Prognosis: Excellent. SURGERY: Temporary waiver due to underage.
