Estimated age: 3-4 weeks based on dentition
Microchip noted on Intake? Scanned negative on intake
History : Finder knows the kitten’s mother and saw the kitten’s eye and took the kitten from mom to bring here
Subjective: Alert, mews, sits for petting and attention
Observed Behavior – Allows all handling
Evidence of Cruelty seen – None
Evidence of Trauma seen – None
Objective
BAR, estimated 5% dehydrated, BCS 1/9, MMs pale/white
EENT: OD large crusted mass over globe with purulent discharge – it appears that the eye ruptured and became infected. Pt shakes head frequently. OS mild crusty yellow discharge. Crusty yellow nasal discharge. Clean ears.
Oral Exam: Decid Is and C’s are partially erupted
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Soft, tucked up, non painful, no masses palpated
U/G: Male kitten
MSI: Covered in flea dirt. Ambulatory x 4, no masses noted.
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally.
Assessment:
1. URI with ruptured OD and severe ocular/periocular infection
2. Emaciated
3. Large amount of flea dirt, suspect anemia
Prognosis: Fair
Plan:
1. Buprenorphine 0.04 ml SQ q8 hours x5 days
2. Erythromycin OU BID x10 days
3. Nebulize this morning
4. Sedate this afternoon to debride OD
5. Fortiflora
6. Doxycycline 0.1 ml PO SID x4 more days
7. Continue 20 ml SQF SID x3 days
8. Expect to enucleate in future, when pt is bigger, older and less flea-ridden

According to feeding log, eating both KMR and cat food. Ate about 2ml each time fed. QAR. Administered 0.04ml Buprenorphine SQ per DVM 1088 and applied erythromycin to both eyes. Will set up in kennel and nebulize this AM.
