Medical Summary DVM Intake Exam Estimated age: ~4-6 weeks Microchip noted on Intake? negative History : stray Subjective: BARH. 3 piles of FS 7/7 diarrhea in cage Observed Behavior – very sweet. Easily handleable and easy to pick up. Did well for all medical handling and procedures Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: kitten dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, diarrhea around perineum CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Underage Diarrhea Plan: Continue to monitor while at BACC Recheck diarrhea in 2 days-if not improved consider adding metronidazole Parvo snap-negative Prognosis: good SURGERY: temporary waiver for surgery due to underage
