Patch is a 5 year old unaltered male white and black DSH mix; he came to his home from a previous owner and was surrender due to the owner moving into an apartment that does not allow pets. He has no known health issues and injures no vet history.
Around strangers Patch is very friendly and outgoing and only lived in a home with one adult. He has been around other male cat and is friendly and playful with the other cat and has no bite history.
Patch isn’t bothered during a bath, and allows people to pet him and hold him with no issues. He struggles to go into the carrier and tries to run out. He was described as friendly, playful, and affectionate with a high activity level. He likes to play with balls, laser pointer, and cat dancer. He is free feed dry food and wet food and is litter box rained with an uncovered box with clumping litter. He has a vertical scratching post with rough rope and carpet.
Upon intake Patch was very friendly he was purring and licked the counselor’s hand, he was collared and a photo was taken.
MEDICAL:

History: Patient is congested and producing eye discharge.
Obj
EENT- left eye is closed and producing alot of purulent discharge.
Skin- No ectoparasite seen
Abdomen- NAF
Ms- Normal muscle mas
Behavior BAR
Neuro- Ambulatory in all 4 limbs
Plan
move to isolation
0.5 inch erythromycin ointment BID OU for 10 days
1.1 ml doxycycline sid po for 10 days.
