Medical Summary DVM Intake Exam Estimated age: 2 months History : stray Subjective: Observed Behavior – friendly Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS EENT: OD- conjunctival hyperemia and chemosis, OS- clear ears clean, nasal congestion but no nasal discharge noted Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment conjunctivitis OD nasal congestion- R/O URI vs other Plan erythromycin OD BID x 7 days doxycycline 8 mg PO SID x 10 days Prognosis: good SURGERY: Temporary waiver due to under 2 lbs and URI
