Medical Summary Microchip: negative, placed 981020021154816 DVM Intake Exam Estimated age: 10-12 y Microchip noted on Intake? negative, placed 981020021154816 History : brought in as a stray, very weak and emaciated on presentation; no other history known; u/a showed 4+ glucose Subjective: QDR, very weak, able to stand only for a few seconds Observed Behavior – no fractious behavioral appreciated initially but very weak Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = P = 100 R = WNL BCS 3/9 EENT: mild anisocoria (slightly mydriatic OS), clouding of lens OU; no nasal/ocular discharge noted; no palpable thyroid slip Oral Exam: mm pk, tacky; CRT >2 sec; upper left canine missing; tartar; some vomitus in mouth PLN: No enlargements noted H/L: NSR, NMA; relative bradycardia; thready pulses; eupnic, no abnormal lung sounds ABD: soft, nonpainful; doughy; no palpable abnormalities U/G: neutered male MSI: very thin BCS, unthrifty haircoat; unable to stand for more than a few seconds, very weak; no obvious fractures or other orthopedic abnormalities; polydactyl on front paws bilat CNS: depressed mentation; ambulatory x 4 but very weak; dec withdrawal on hindlimbs bilat Rectal: not performed Assessment 1. Diabetes mellitus 2. azotemia-r/o dehydration vs renal dz 3. ear mites 4. anisocoria-likely due to severe hypernatremia Plan Place IVC-give 75 ml bolus LRS then continue on 12 ml/hr CBC/chem Tresaderm AU SID x 10 d give insulin 1U SQ, recheck BG 1 hr after start ampicillin 22 mg/kg IV TID Prognosis: guarded
