New URI noted on rounds Subjective: QAR. No c/v/d but is congested and sneezing with sneeze marks in cage. Eating well. Objective EENT: no nasal or ocular d/c H/L: audible upper respiratory congestion and sneezing Assessment: URI Hx Otitis externa Plan: Start doxycycline 10mg/kg PO SID x10d Recheck at day 5, 10 Move to iso Prognosis: Good
Recheck URI-day 5 treatment Subjective: BAR. No c/v/d but is sneezing. Eating well and very vocal. Audibly congested. Objective EENT: no nasal or ocular d/c H/L: audible upper respiratory congestion and sneezing Assessment: URI -improving Hx Otitis externa Plan: Continue doxycycline 10mg/kg PO SID Recheck at day 10 Iso Prognosis: Good

9/15/17 DVM Intake Exam Estimated age: 2-3yrs Microchip noted on Intake? no History : Stray – Unknown Subjective: Cat is Q/BARH. Observed Behavior – Requires restraint throughout exam. Hissing and growling, escalates to swatting when LVT tried to give Pyrantel (will put in food). Screamed for blood draw. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = wnl R = wnl BCS 4/9 EENT: Eyes clear, ears dark debris AU, no nasal discharge noted Oral Exam: Grade 0-1/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: no masses palpated U/G: both testicles palpable scrotally MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Otitis externa, AU (mild) Plan: Recommend cleaning ears while under anesthesia for neuter. Acceptable candidate for adoption or rescue pending behavior eval. Prognosis: Good SURGERY: Okay for surgery
