EENT: Eyes clear, ears clean, Mild serous nasal discharge- increased respiratory noise heard on inhalation
Oral Exam: Mild tartar, gingivitis, MM pink and moist
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Intact male- 2 descended symmetrical testicles
MSI: Ambulatory x 4, skin free of parasites, no masses noted, Matting right axillary region otherwise WNL
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: URI, Gingivitis
Plan: Doxycycline 50 mg/ml – 0.60 ml PO SID
Prognosis: Excellent with medications
SURGERY: Temporary waiver due to URI

Haze is a 3 year old orange and white tabby cat. He came to the center as a stray. During intake, he started to purr and head butt the counselor. Haze was very friendly and allowed all handling. He allowed to be scanned, collared, and photographed.
