DVM Intake Exam Estimated age: ~8yrs Microchip noted on Intake? no History : Stray Subjective: BARH Observed Behavior – Allowed all handling but appeared nervous, would try to get away but allowed to be picked up and placed back on table calmly Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = WNL R = WNL BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Mild to moderate dental tartar, pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, harsh lung sounds in all fields, eupnic ABD: Non painful, no masses palpated U/G: Female MSI: Ambulatory x 4, skin free of parasites, pudunculated tumor stemming from left caudal mammary gland, lobulated, ~3-4 x 1cm, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Senior pet, mammary gland tumor Plan: Scheduled CBC/Chem, radiographs. Continue to monitor while at BACC Prognosis: guarded for malignant mammary gland tumor SURGERY: Permanent waiver due to age/tumor

[Spay/Neuter Waiver – Age] It is the policy of ACC not to perform surgery on any animal over the age of 8-10 years due to the higher risks incurred in a shelter setting. The veterinarian is hereby issuing a permanent spay/neuter waiver, from the spay/neuter requirements of the City of NY due to the estimated age of this animal. ACC does recommend you consult with your veterinarian to determine if surgical sterilization is appropriate.

Eating wet food and tuna well. Resting comfortably in cage but is at the back of the cage and very nervous. Objective EENT: Eyes clear, no nasal discharge noted H/L: Eupneic, normal RR/RE MSI: Ambulatory x 4 CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric Mammary gland tumor Decreased appetite-resolved Plan: Continue to monitor while at BACC Extend simbadol 0.24mg/kg SQ SID x3d until 1/27 Recheck 1/27 Continue tresaderm AU BID until 2/4 Prognosis: guarded for malignant mammary gland tumor

KNOWN HISTORY:: Neon was brought in as a stray, so there is no information on their behavior history or tendencies in their previous home.
MEDICAL BEHAVIOR:: 01/19/18 Observed Behavior – Allowed all handling but appeared nervous, would try to get away but allowed to be picked up and placed back on table calmly
ENRICHMENT NOTES:: 01/22/18 Lying in back of kennel, body low and tense and ears tilted. She tolerates petting initially, then begins to warm up after a few passes along the back. Raises her rear with each pet as ears shift forward. Begins to purr. Timid, very sweet once approached. Beginning to warm up! Did well today. Not too interested in treats. 01/23/18 Lying in back of kennel, body low and ears forward. Makes eye contact when spoken to and scent marks blanket, eyes soft. She raises her head as I reach towards her and leans in for petting, allowing touch along her head and body. She raises her rear and tail as my hand passes along her back and steps forward. Eats treats offered. Showing improvement! Lowers body back down after interaction has ended – focused on environment.
Cage Condition:: Cage is neat
Reaction to assessor:: Neon was lying down by the back with a low body posture.
Reaction when softly spoken to:: Neon presses her face on the cage wall, but looks back at the assessor and blinks softly.
Reaction to cage door opening:: Neon remains in place and remains calm.
Reaction to touch:: Neon sniffs the assessor’s hand and pushes up against it once touched. She leans in for cheek and chin rubs, begins to purr, and lightly lifts her rear.
Reaction to being picked up:: Neon is a bit tense during pick up, but remains calm when held.
